Ways for Parents to support Crush Ultimate
Registration for 2023 consists of five steps, done primarily online
Step 1 - Complete the 2023 Crush Ultimate Sign-Up Form. Coaches use this to coordinate teams and set up communication with players and parents.
Step 2 - Register for Ultimate Frisbee through the SLPHS Clubs and Activities page on the high school website: https://hs.slpschools.org/athletics-activities/student-clubs-activities. Online registration and waivers must be completed before players are allowed to practice, including pre-season.
Step 3 - Register for MN Ultimate youth membership at https://minnesotaultimate.org/. MN Ultimate membership ($40) is required to play in all league games and at all tournaments. Online waivers must be completed during registration and any player under 18 must complete a Medical Authorization form, print it, and bring it to the coaches by the first regular season practice. Step 4 - Pay the $175 team dues directly to "SLPHS Ultimate" by check or PayPal (Crush Ultimate PayPal Link) before March 1st. Checks should be given to your coach in an envelope clearly marked with the player's name. Team dues do not include jerseys/shorts, cleats, or MNU membership.
Step 5 - Read, Print, and Sign the SLP Ultimate Behavioral Agreement and turn it into your coach with the other registration information (SLP Ultimate Behavioral Agreement).
